Internal medicine looks after the illnesses that do not belong to one organ: prolonged fever, unexplained weight loss, cough, breathlessness, weakness, anaemia, thyroid and hormonal problems, and the long-term management of diabetes, blood pressure, asthma and kidney or liver disease that does not need an operation. This is also the department that coordinates care when several problems are present at once.

Who runs this clinic

General medicine at R.R. Healthcare is a consultation-led clinic: a physician who works up the complaint rather than routing it, with the laboratory, digital X-ray and ultrasound along the same corridor, so the tests a case needs can often be read before the visit ends. The hospital publishes a consultant profile only once it has confirmed it and the consultant has cleared it for publication, so the name on this week’s internal-medicine board is given by the appointment desk rather than printed on this page.

The long-run half of this clinic

A large share of the work here is not a single complaint but a set of numbers that need holding: type 2 diabetes that has stopped answering the first tablets, and the pressures that travel with it — kidneys, eyes, feet, nerves, cholesterol. The point of a follow-up visit is not a repeat prescription; it is the review of the last three months’ readings, a check of what the medicines are doing, and a change made before something symptoms appear.

Your visit, step by step

  • Book as internal medicine (or general medicine). If a previous doctor wrote a referral, bring it — it tells us where the work already stopped.
  • Bring the record trail: old reports, the last HbA1c or creatinine, the blood pressure log from home if you keep one, and every medicine including the ones bought without a prescription.
  • Examination, then tests chosen for you. Laboratory work is done in the hospital’s own 24×7 lab, so many reports come back inside the same visit.
  • A plan on paper: what changed, what to watch at home, when to come back, and which warning signs should send you to casualty instead.

What to bring to a medicine consultation

  • The last HbA1c, creatinine, thyroid or lipid report you can find, even if it is months old and from another laboratory. A number on its own means less than the same number twice.
  • Every medicine and every supplement, in their strips. Herbal preparations and over-the-counter painkillers count, because they are the two things most often missed on a written list.
  • A home record if you keep one — blood pressure readings, sugar readings, weights, or simply the days you felt unwell.
  • For a prolonged fever: a note of when it started, whether it comes and goes at a particular time of day, and any antibiotic already taken for it and for how many days.

Questions the desk gets asked

That is the point of this clinic. Diabetes and blood pressure and a thyroid problem and a bad knee usually arrive together in the same sixty-year-old, and the medicine consultant’s job is to hold them in one plan rather than send the patient to four rooms with four sets of tablets that nobody has read together.

When a problem needs a specialist or a facility the campus does not run, the doctor names it at the visit and says why. Being referred out is not a failure of the clinic and it is not hidden from the patient; the alternative — keeping a case that needed someone else — is worse.

The follow-up date is written on the plan, and the warning signs that should send you to casualty instead of waiting for it are named at the same time. If you have got home and cannot remember which was which, telephone the desk — a question is cheaper than a missed week.

Where this clinic sits in the hospital

Internal medicine is the department most other departments route through: the laboratory is in this building, so reports usually come back inside the visit, and paediatrics and orthopaedics sit on the same campus. For the unwell patient who arrives without an appointment at all, casualty is open round the clock, and the health packages list what can be checked when there is no symptom yet.

Fever with confusion or breathlessness, chest tightness, unable to keep fluids down for a day, very low blood sugar with drowsiness or sweating, blood sugars persistently above 300 mg/dL with vomiting, a swollen painful calf, or black stools: use casualty rather than waiting for the next OPD day.
